Denied claim recovery for independent practices
Your practice wrote off money it was owed.
Most denied claims are never appealed — chasing a $400 denial costs more in staff time than it brings back, so it ages out and disappears. We work those claims for you on contingency. No software to learn, no setup fee, no monthly cost.

Why this money exists
Denials are rising, and almost nobody appeals them.
Sources: KFF analysis of HealthCare.gov marketplace data, 2024 (denial rate, appeal rate, and the 34% internal-appeal overturn rate); Revecore 2026 denials & underpayments analysis (57% overturn rate, Medicare Advantage); payer-published appeal deadlines. Marketplace and Medicare Advantage figures measure different populations and are not directly comparable. These are industry-wide statistics, not a prediction of results for any individual practice.
How it works
Four steps. About twenty minutes of your staff's time.
Send a denial report
Your biller exports denials from your practice management system — a standard report that takes about ten minutes to pull.
Get a free written review
We grade every claim: recoverable, fixable, or genuinely dead — with a dollar estimate and the deadline on each one. Free, no obligation.
We work the claims
Corrections and resubmissions where the denial was a technical error. Formal appeals with documentation where it wasn't. You sign; we prepare and file.
The insurer pays you
Recoveries land in your account, exactly as they normally would. We invoice our percentage monthly, with a claim-by-claim report.
What the review finds
Most denials aren't judgment calls. They're paperwork.
A large share of denials are technical — a missing modifier, a mismatched member ID, an authorization that existed but was never attached. Those don't need an argument; they need a correction and a resubmission. The rest are medical-necessity and authorization disputes, which need documentation and a real appeal.
We sort your denials into exactly those groups and tell you what each is worth before you commit to anything.
- Missing or incomplete information (CO-16)
- Modifier missing or inconsistent (CO-4)
- Member ID / name mismatch (CO-140)
- Coordination-of-benefits issues (CO-22)
- Authorization absent or exceeded (CO-197, CO-198)
- Medical necessity disputes (CO-50)
- Visit frequency denials (CO-151)
- Bundling and NCCI edit disputes (CO-97)
Pricing
You cannot lose money on this.
We charge a percentage of what we actually recover, invoiced monthly against the remittances you receive. If a claim doesn't get paid, there is no fee for that claim. There is no subscription, no minimum, and no setup cost — so the worst realistic outcome is that you spent twenty minutes exporting a report.
Rates depend on claim age and mix; older written-off claims carry a higher rate than ongoing denial flow because they're harder to win. We'll quote you a firm number with your free review.
What we don't do
- We don't replace your biller or your billing company
- We don't require you to change practice management systems
- We don't contact your patients, ever
- We don't collect patient balances or handle patient debt
- We don't ask for exclusivity on your billing
- We don't lock you into a term — 30 days' notice, any time
Security & compliance
Handled the way your compliance officer expects.
Working your denials makes us a HIPAA business associate. We sign a Business Associate Agreement before receiving any patient information, and we ask only for the minimum necessary to work a claim.
- Signed Business Associate Agreement before any data changes hands
- Encrypted transfer and storage; access limited to claim work
- Minimum-necessary principle — we don't need charts we aren't appealing
- All vendors in our workflow operate under their own BAAs
- Claim-level audit trail of everything submitted on your behalf
- Your data returned or destroyed when we stop working together
Questions
What practices ask us first
We already have a billing company. Does this conflict?
No. Billing companies are paid a percentage of total collections and are built to keep clean claims flowing — denials are the slow, unprofitable work that tends to get deprioritized. We work only the claims that came back denied. Most practices keep everything exactly as it is and simply stop writing off the denials.
What exactly do you need from us?
To start: a denial report from your practice management system, which is a standard export. If we move forward, either monthly exports or a limited user account on your system and payer portals so we can file and track. We ask for the minimum necessary and nothing beyond claim work.
How do you get paid, and how do we verify it?
The insurer pays you directly — money never routes through us. Each month we invoice our percentage of recoveries with a claim-by-claim report you can match line-for-line against your remittance advice. If you can't verify a recovery, you don't pay for it.
Will appealing hurt our relationship with payers?
Appeals are a normal, contractual part of claims adjudication — payers process millions of them a year. We file professionally and within each payer's stated process. We don't file frivolous appeals, because our economics depend on winning, not on volume.
How far back can you go?
It depends on the payer. UnitedHealthcare allows 65 days from denial to appeal; most commercial plans allow 180; Medicare Advantage is typically 60–65. Corrected claims often have a longer window than formal appeals, so some older technical denials are still recoverable. The free review tells you exactly which of your claims are still inside their window — and which already aren't.
What does the free review actually cost us?
About ten minutes for your biller to export a report. You get back a written analysis of what's recoverable and what it's worth, whether or not you hire us. If the number is too small to bother with, we'll tell you that.
